2022 Participant Forms

Once you have registered and been assigned to a team and coach, please see the instructions below:

1) Please access the portal below to E-sign the Parent Consent form, Code of Conduct form, and Covid19 waiver. You will also have the opportunity to upload your player's photo.

2)  If you are new to CFL, you must supply your child's ORIGINAL birth certificate along with a copy. The original will be returned once reviewed by the executive board. 

ALL paperwork will need to be given to your Head Coach or Team Parent by 06/15/2022
